Object to a Name on the Voters List
Use this service if you are a registered voter and believe that another person should not be included on the provisional or preliminary list of voters for your electoral district.
You can submit Form No. 4 – Notice of Objection to the Supervisor of Elections stating why you believe the person is disqualified from being included on the voters list.

Use the Applicant Personal Information Form when submitting voter registration, transfer, claim or objection forms to the Office of the Supervisor of Elections.
