Amendments to the Birth Record – Correction of Error/Omission

This service is for customers who's name is incorrect on BirthPaper or wants to admend their legal name on their birth record.

Process

Step 1:  Customer submits an affidavit requesting to correct the Birth record.

Step 2:  Fee is collected from the Customer.

Step 3:  A receipt is issued to the Customer.

Step 4:  Affidavit is log and submitted to the Registrar General for review and approval.

Step 5:  Customer is contacted to collect the revised Birth Certificate.

$20.00 Birth Certificate
 

Supporting Documents

  • Affidavits from both parents ($1.00 postage stamp affixed)
  • Previous Birth Certificate.

Additional Notes

Staff will ensure that all necessary particulars in the affidavit are included in the request such as, $1.00 stamp, supporting documents e.g. ID, and must surrender any Birth Certificate previously issued.
