Applicants must provide the following:
- Completed Duplicate of Livery Permit application form.
- Two passport-sized photographs.
- A valid passport or driver's licence for identification.
- Existing livery permit number.
- Name of employer.
- Original permit issue date and renewal date.
- Payment of the $100.00 duplicate permit fee.

How to Apply
Step 1: Obtain the Application Form
Download the Duplicate of Livery Permit application form from this service page.
Step 2: Complete the Application
Provide your full name, livery permit number, employer's name, physical address, telephone number, original date of issue and renewal date.
Indicate whether the permit was lost, destroyed or mutilated. Sign and date the application.
Step 3: Prepare Supporting Documents
Ensure you have two passport-sized photographs and your passport or driver's licence.
Step 4: Submit Your Application
Submit the completed application form and supporting documents to the Department of Motor Vehicles for processing.
Step 5: Pay the Applicable Fee
Pay the required $100.00 fee for the duplicate livery permit.
