Step 1: Obtain the Application Form
Download the Duplicate of Taxi Permit application form from this service page.
Step 2: Complete the Application
Provide your full name, taxi permit number, physical address, telephone number, original date of issue and renewal date.
Indicate whether the permit was lost, destroyed or mutilated. Sign and date the application.
Step 3: Prepare Supporting Documents
Ensure you have two passport-sized photographs and your passport or driver's licence.
Step 4: Submit Your Application
Submit the completed application form and supporting documents to the Department of Motor Vehicles for processing.
Step 5: Pay the Applicable Fee
Pay the required $15.00 fee for the duplicate taxi permit.
