Who Can Apply

You can use this service if you:

  • Have lost your driver's licence and need a replacement.
  • Have legally changed your name and need your driver's licence updated.

What You Need

For a Lost Driver's Licence:

  • Completed Duplicate Driver's Licence Form.
  • A valid form of photo identification.
  • Your driver's licence number, as requested on the form. Contact the Department of Motor Vehicles if you do not know your driver's licence number

For a Name Change:

  • Completed Duplicate Driver's Licence Form.
  • Copy of your photo identification.
  • Your existing driver's licence.
  • Supporting document showing your legal name change, such as a marriage certificate or legal name change document.

How to Apply

Step 1: Get the application form

Download or collect the Duplicate Driver's Licence Form from the Department of Motor Vehicles.
 

Step 2: Complete the form

Enter your name, address, driver's licence number and telephone number. Sign and date the form.
 

Step 3: Attach your identification

Include a copy of your photo identification with the completed form.
 

Step 4: Submit your application

Take the completed form and required identification to the Department of Motor Vehicles.
 

Step 5: Pay the replacement fee

Pay the $15.00 fee for your replacement driver's licence.
